New Client Forms and Procedures

New clients please read and complete the information below to help us make your visit as helpful, enjoyable, and efficient as possible:

Please download, print, and fill out the CHILD NEW PATIENT FORM for patients under the age of 18, or the ADULT NEW PATIENT FORM if you are a patient over the age of 18, along with the corresponding CONSENT FOR TREATMENT FORM, PAYMENT INFORMATION FORM, COMMUNICATION POLICY FORM and RELEASE OF INFORMATION FORM. Please bring them into The Anxiety Treatment Center of Greenwich on your first visit.

While we do not bill insurance directly, we encourage you to explore your out of network benefits. Our services are generally reimbursed by health savings accounts.
